Procedure:
Rhinoplasty with ear cartilage graft to reconstruct
the left middle nasal vault.
Patient
Concerns: This 46 year old female complained
of difficulty breathing on the left side of the nasal
passage. This patient had a past history of nasal trauma
and underwent septal reconstruction three years earlier
under the care of another surgeon. Since that time,
she continued to experience symptoms of nasal obstruction
on the left side of the nose and has difficulty breathing.
In addition, she has noted an external nasal deformity.
The external nose was displaced to the left and there
was narrowing of the middle portion of the nose on the
left. There was nasal tip asymmetry and minimal tip
support.
Dr.
Markus’ Treatment: Due to prior surgery
by another physician, the patient’s septal cartilage
was unavailable for reconstruction. As a result, pieces
of the patient’s ear cartilage were employed.
The breathing space was opened by placing an ear cartilage
graft on the left side. Nasal tip projection was increased.
Pieces of the patient’s own ear cartilage were
employed to increase tip projection and sculpture the
tip. In addition, the nasal pyramid was straightened.
